Based on the previous works of the authors and the concept of multilevel natural selection of E. Mayr, an evolutionary-ontogenetic approach to the development of the epistemology of clinical oncology has been proposed. This approach includes Darwinist and Lamarckist models of evolution and does not conflict with all experience in the development of clinical oncology. It will allow to improve the effectiveness of clinical thinking in the field of oncology. We assume that it is applicable for the development of both the prevention and treatment of cancer based on the individuation of doctoring. During that physician can involve psychophysiological determinants of human cultural evolution.
